I wonder if anyone knows who made this bear, or which pattern was used for it.
Would love to find this or something very similar,

That looks like a Monica Spicer bear to me.  She has many patterns for sale if you check her website she should have one close to that one.  Just google her name and you should find what you need.

Definitely one of Monica's. She has a pattern called Dufus which is very similar juts with an open mouth. Its an applied open mouth so you could just leave it off to get that look. http://www.monicasattictreasures.com.au/dufus%20p.html
